Have you ever been asked to authorise and sign a PGD? Would you know how to write one or what should be included in a PGD? Pharmacy Consulting Limited was asked to authorise a clients PGDs this month.
General Points to remember:
- Am I insured to sign off a PGD? And am I competent?
- PGDs should be in Plain English concise and easy to understand
- Clinical content should be backed up with references e.g. SPC, British National Formulary ( BNF) or Patient Information Leaflet (PIL)
- Use hyper links to essential articles
- Clinical accuracy, spelling and layout should be checked
Authorisation should include:
- Lead Doctor
- Lead Nurse or allied Health Professionals
- Lead Pharmacist
- Clinical Governance Lead
- And any Organisational Lead or RP
All their comments must be recorded
The PGD should include:
- The Drug Name and Class
- Clinical information e.g. indication, inclusion criteria cautions and exclusion criteria
- Drug details e.g. Name, Form,Strength,Dosage,Treatment Period etc.
- Referral Protocols
- Records and audit trail
- Staff Qualifications, competencies and CPD
At the back all the Individual authorisations Peer reviews are attached and the date to review the PGD.
